Foundation route for the planned refer.soraheadache.com experience. Same referral intake contract as the legacy public provider form — fax and web referrals converge on one pipeline when connected to production.

Do not enter real PHI. There is no live fax, SMS, email vendor, NPPES lookup, DNS for refer.soraheadache.com, or HealthLake write from this path. Submissions stay in-memory on the dev server unless you separately POST to the staff intake API.
Target hostname (architecture): refer.soraheadache.com — converges with fax referrals into the same internal referral intake + review pipeline when wired to production.
